RN Care Manager: Care Coordination & Outcomes Leader

Molina Healthcare in Biloxi, MS is seeking a dedicated care manager RN to support care management and coordination across the continuum. You will assess, plan, implement, and monitor member care with a multidisciplinary team to improve outcomes and cost-effective care.

You will conduct visits (telephonic, in person, or home), manage a caseload, promote integration of services including behavioral health and LTSS, and participate in ICT meetings while navigating EMR and HIPAA requirements.
