RN Care Manager — Care Coordination & Discharge

UNC Health is seeking an RN Care Manager at UNC Medical Center in Chapel Hill. The role focuses on comprehensive care coordination, risk assessment, and patient-centered care planning within a multidisciplinary team.

The Care Manager will guide safe discharge, ensure effective care transitions, and advocate for patients throughout their healthcare journey. The schedule features 40 hrs/week on five 8-hour shifts, with weekend and holiday rotation, no nights, and no on-call.
