In this role, you will perform comprehensive assessments, develop and implement care plans, facilitate transitions of care, and collaborate closely with physicians, interdisciplinary teams, patients, and families. You will monitor outcomes, manage utilization, support patient education, and ensure adherence to evidence-based guidelines while advocating for patient needs.
