RN Care Manager

Role Overview

The RN Hospital Care Manager I provides comprehensive care management for designated patients, utilizing clinical expertise to conduct screenings, assessments, and evaluations. They develop and implement patient-centered care plans, collaborating with patients, families, healthcare providers, payers, and community resources to ensure efficient, high-quality care across inpatient and outpatient settings.

Key Responsibilities

• Assess patients through screenings and evaluations.


• Develop, adjust, and coordinate individualized care plans addressing medical and social needs.


• Support patient self-management via motivational interviewing and coaching.


• Educate and advocate for patients and healthcare teams to promote self‑care and rights.


• Collaborate with multidisciplinary teams to ensure timely, effective care.


• Lead process improvement initiatives, enhance care transitions, and ensure policy adherence.


• Provide education on goals of care for informed decision-making.


• Track key metrics, analyze trends, and ensure compliance with regulations.

Qualifications & Skills

• Current RN license in the practice state.


• Bachelor of Science in Nursing (BSN); must obtain BSN within four years if hired without one.


• Clinical nursing experience in inpatient hospital settings, with knowledge of hospital processes.


• Understanding of disease management, treatment, barriers to care, and length of stay.


• Skills in assessment, care planning, motivational interviewing, critical thinking, communication, time management, and patient education.


• Preferred: Case management certification, experience in discharge planning, utilization review, or value-based care; basic computer proficiency.

Physical & Other Requirements

• Ability to see, read, communicate effectively, and manipulate equipment accurately.


• Handle high-stress situations, make quick decisions, and manage multiple tasks.


• For roles involving driving, ability to operate a vehicle safely.
