RN Care Manager (Registered Nurse)

Altera Health & Rehabilitation Center is seeking a compassionate and experienced RN Care Manager to join our clinical leadership team in Fairhope, Alabama .

In this role, you will coordinate resident care from admission through discharge, collaborate with the interdisciplinary team, and support quality outcomes for our short-term rehabilitation and long-term care residents.

What You’ll Do

• Coordinate resident care from admission through discharge
• Collaborate with physicians, nursing, therapy, admissions, and social services
• Lead interdisciplinary care plan meetings
• Assist with discharge planning and transitions of care
• Coordinate care for Medicare, managed care, and long-term care residents
• Communicate with residents and families about care plans and progress
• Maintain accurate, timely clinical documentation and regulatory compliance
• Support quality improvement initiatives and help reduce avoidable hospital readmissions

Qualifications

• Current Alabama Registered Nurse (RN) license required
• Experience in skilled nursing, rehabilitation, long-term care, case management, MDS, admissions, or care coordination preferred
• Strong clinical assessment, communication, and organizational skills
• Ability to manage multiple priorities and work collaboratively with an interdisciplinary team
