RN Care Line - MN Licensed

Providing triage and health advice by phone, the part-time RN Care Line - MN Licensed will utilize evidence-based protocols to assess patient symptoms, process prescription refill requests, and coordinate patient transfers in a remote setting. Key responsibilities Deliver triage and health advice to patients, utilizing computerized protocols for symptom assessment Process prescription refill requests from pharmacies and patients while documenting all calls electronically Assist with the coordination of patient transfers and participate in transitional care calls for discharged patients Required qualifications Current RN license in Minnesota, North Dakota, and/or Wisconsin, with the ability to obtain all three within three months of hire Minimum of 2 years of experience in telephone triage, transitional care management, or remote patient monitoring Experience with ambulatory computer software systems Availability for day/evening shifts, including weekends and holidays
