Description: Schedule: Hybrid position - 10% onsite, 90% remote.40 hours a week, Monday to Friday, 8:00 AM - 5:00 PM.No weekends or holidays.The Registered Nurse will complete a report for each member in the required format, documenting the medical necessity for skilled services available to meet individualized needs.In-person assessments every 90 days and as needed.The position is field-based, requiring travel to complete in-person assessments.Completes nursing referrals and places vendor calls.Completes special projects as needed.Completes documentation within established timeframes.Engages in interprofessional collaboration internally and externally.Participates in multidisciplinary planning meetings to determine and identify appropriate services per established policies and guidelines.Excellent communication skills, including the ability to explain complex information and educate patients and families.Collaborates with members and their support systems.Attends mandatory meetings.Completes required annual training.
