RN Care Coordinator – Outpatient Case Management

The Judge Group is seeking an RN Care Coordinator for an onsite outpatient setting in Houston, TX. This role supports patients and providers by coordinating care plans, triaging clinical needs, and ensuring clear communication across the care team.

Requirements include 3–5+ years of RN experience, a family medicine background, active RN license, and EMR proficiency (Epic or similar). Schedule is Monday–Friday, 8:00 AM–5:00 PM, in a fast-paced environment.
