Job Description
We are looking for a dedicated Community-Based Support Specialist to provide essential support to individuals with Mental Health and Intellectual/Developmental Disabilities (MH/IDD). This is a PRN position, with opportunities to work up to 24 hours per week.
Responsibilities
- Assist and support individuals with mental health challenges and developmental disabilities in a community-based setting.
- Implement person-centered plans tailored to each individual's needs and goals.
- Monitor and document progress in accordance with organizational policies and protocols.
- Collaborate with a multidisciplinary team to foster positive outcomes for clients.
- Provide crisis intervention and support as necessary to ensure the safety and well-being of clients.
Required Experience / Certifications / Licensure
- Experience working with individuals with Mental Health and Intellectual/Developmental Disabilities is preferred.
- A relevant certification or training in mental health or developmental support is desirable.
