In this role, you will:
- Conduct client assessments and develop care plans
- Visit clients and support ongoing care needs
- Communicate regularly with families and providers
- Provide guidance during times of transition or change
- Provide occasional weekend on-call support
We’re looking for someone who:
- Has experience in geriatrics, home health, hospice, or related fields
- Is highly organized and comfortable managing multiple moving parts
- Is comfortable using technology (e.g., Google Workspace, care management systems)
- Builds strong relationships and communicates thoughtfully
- Demonstrates emotional intelligence and healthy boundaries
Requirements:
- CCM or CMC certification is a plus
- Ability to travel for in-person client visits in the D.C. Metro area
