After Hours RN (DuPage Territory-Weekend)The After Hours RN is responsible, as a member of the interdisciplinary team, for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers consistent with Lightways Hospice and Serious Illness Care policies and procedures. The RN plans and directs services to meet the needs of individuals and families/caregivers within their homes and communities.Key Responsibilities:Provides after-hours skilled nursing care, assessments, and interventions to hospice patients in their homes, ensuring comfort and quality careCompletes comprehensive patient evaluations, develops and updates care plans, and collaborates with the interdisciplinary teamResponds to urgent patient needs, including medical and emotional crises, and communicates with physicians, triage, and care teamsAdministers medications, educates patients and families, and documents all care accurately and timelyMaintains compliance with regulations, completes required training, and supports overall continuity and coordination of patient careMonday- Friday with weekend and holiday rotation.Education/Experience:Current RN licenseMinimum of 3 years RN experience in home health, critical care, or long-term careValid driver's license and insured, reliable vehicleBenefits: (Full-Time)401(k) matchingHealth InsuranceDental InsuranceVision InsurancePet InsurancePersonal Time OffHoliday Paid
