Resource RNLightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.The Resource RN is a float position that is responsible for providing skilled nursing intervention and supportive care in patients' homes, nursing homes or long term care facilities. This RN administers medications and treatments as prescribed by the physician and plan of care.
This position works 2(8 hour) shifts and 2(12 hour) shifts rotating weekends and holidays.Key Responsibilities:Conducts comprehensive patient assessments and develops, updates, and manages individualized care plans for hospice patientsProvides skilled nursing care, including medication administration, treatments, and patient/family educationCollaborates with the interdisciplinary team and participates in IDT meetings to ensure coordinated patient careSupports patient needs through on-call coverage, crisis response, and assistance with daily living activitiesEnsures compliance with regulatory standards, maintains accurate documentation, and meets quality and performance metricsRequirements:RN with a minimum of two (2) years nursing experience, at least one of which is in the area of public health, home care or hospice nursing preferredIncumbent must be self directed and able to work with minimal supervisionMust be able to work 40 hours a week with a minimum of 20 hours on call/month, may increase based on staffingEducation/Experience:Current RN licenseMinimum of 2 years' experience, at least 1 year area of public health, home care, or hospice nursing is preferredValid driver's license and insured, reliable vehicleBenefits:401(k) matchingHealth InsuranceDental InsuranceVision InsurancePet InsurancePersonal Time OffHoliday Paid
