FLSA: Non-Exempt, 4 - .9FTE's (36 hrs/wk) & 2 - .2FTE's (8 hrs/wk)
PAY GRADE: B08 ($25.19 - $36.97 per hour, depending on experience)

Job Summary

Provides residential care and support for youth, promoting recovery through mental health services, rehabilitation, social‑emotional skills development, and personal services. Supports clients with activities of daily living (ADL), safety monitoring, and participation in therapeutic and recreational activities.

Responsibilities

• Provide 24‑hour supervision, ensuring safety and welfare of residents.
• Document daily clinical information, progress notes, treatment plan updates, monthly summaries, and classroom activity records.
• Complete incident reports, medication administration records, and other required documentation in accordance with policies and OARs.
• Facilitate skills and activity groups oriented to treatment needs.
• Assist with client intake packets, guardianship signatures, and coordination with educators, providers, and family members.
• Maintain accurate records in binders and the CCS electronic health record (EHR) system.
• Support residents with ADL tasks, personal hygiene, grooming, and transportation within the facility.
• Administer prescribed medications following physician orders, nurse policies, and documented procedures.
• Participate in evacuation/fire drills, seclusion and restraint practices as needed.
• Conduct inventory of supplies and personal belongings; order and purchase program supplies.
• Maintain clean, sanitary, and safe conditions throughout the facility.
• Report safety hazards, violations, and any abuse or neglect in accordance with mandatory reporting policies.
• Attend staff meetings, training sessions, and quality improvement activities.
• Work one‑on‑one with youth to develop basic life skills and support progress toward treatment plan goals.
• Track billable services and complete discharge summaries.
• Other duties as assigned.

Qualifications

Ability to interact respectfully with youth, staff, and supervisors; strong verbal and written communication skills; understanding of laws, regulations, and agency policies; ability to manage crisis situations.

Education and Experience

• High school diploma or equivalent plus a bachelor’s degree in a social services field, or a combination of three years of relevant experience.
• Preferred: three years of experience or education in psychology, counseling, or a related field, and a combination of coursework and practical experience.
• Preference for MHACBO QMHA certification at time of hire; must obtain certification within 30 days of employment.

Additional Skills and Abilities

• Handle highly confidential information with adherence to legal and agency regulations.
• Functional knowledge of business English, medical terminology, and Microsoft Office.
• Good spelling, basic mathematics, and computer skills for EMR data entry.
• Ability to represent the agency positively in the community and collaborate with diverse teams.
• Effective written and oral communication skills.
• Strong organizational, time‑management, and task‑learning capabilities.

Criminal Background Checks

Must pass criminal history checks required by ORS 181.536‑181.537 and OAR 410‑007‑0200 through 410‑007‑0380, along with monthly checks against federal and state exclusion lists.

Pre‑Hire Drug Screening Required

All candidates must complete a pre‑hire drug screen as part of the hiring process.

Personal Auto Insurance

Must hold a valid driver’s license and personal auto insurance for any privately owned vehicles used for CCS obligations. Proof of liability coverage of $300,000 for bodily injury and $100,000 for property damage is required.

Physical Demands

The role requires regular presence on a flexible schedule, physical activity such as lifting and transferring clients, and the ability to work overtime, weekends, evenings, or overnight shifts as needed.

Work Environment

Work occurs in office/home settings and in community environments, including residential homes. Noise levels vary, and travel to meetings and trainings may be required. The position may involve unpredictable community settings with limited handicap access.
