Remote VP, Transitions of Care & Post-Discharge Ops

Urrly, Inc. seeks a VP, Clinical Operations to lead transitions of care and build a new post-discharge service line for Medicare Advantage, Medicaid, and Exchange members. This remote U.S. role reports to the Chief Clinical Officer and offers a $190,000–$220,000 base plus bonus and meaningful equity participation.

You will design the care pathway, scale clinical teams, and own outcomes for readmissions, PCR, and HEDIS-Stars while partnering with payers to optimize ROI and care quality.
