Vaya Health is seeking a Transition & Housing RN for Alamance County, NC. This home-based, remote role requires NC residency or within 40 miles of the NC border, serving multiple NC counties with occasional travel to communities.
The RN will assess, plan, educate, and coordinate care for adults with complex health needs transitioning from ACHs into the community, partnering with hospitals, EMS, and PCPs to ensure safe, person-centered care.
