Humana is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of Humana's home-based services. This remote role reports to the Manager, Care Management and focuses on post-acute levels of care, including home health, hospice, and palliative care.
You will work Monday through Friday 9am-5:30pm EST with additional weekend coverage every 3rd weekend and rotating holidays, and you may train outside the advertised shifts.
