Remote Telephonic Nurse Care Manager

Humana Inc. is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of Home Health, Hospice, and Palliative Care.

This work-from-home role reports to the Manager, Care Management and focuses on ensuring optimal continuity of care across post-acute services. Responsibilities include clinical care determinations, referral facilitation, patient education, and collaborating with the clinical team to improve patient outcomes.
