Working remotely in a full-time capacity, the Remote RN Case Manager will coordinate patient care, develop care plans, and collaborate with healthcare providers to ensure optimal patient outcomes. Key responsibilities Coordinate and manage patient care by assessing needs and developing tailored care plans Collaborate with healthcare providers and patients to ensure effective communication and continuity of care Monitor patient progress and adjust care plans as necessary to achieve health goals Required qualifications Active RN license in the state of practice Minimum of two years of clinical nursing experience Experience in case management or care coordination preferred Certification in case management (CCM) is a plus Proficiency in electronic health records (EHR) systems
