Sentara Health Plans is seeking an Integrated Nurse Case Manager/Registered Nurse (RN) for chronic/complex conditions. This full-time, remote role requires travel for home visits 2–3 times per week in Fredericksburg, VA and surrounding areas.
Standard hours are 8am–5pm EST, Monday–Friday, with residency in Fredericksburg area. You will develop care plans, perform comprehensive assessments, and coordinate care across clinical and social services.
