Managing an assigned patient panel, the full-time Remote RN Care Manager will provide comprehensive care management, establish empathetic relationships with patients, and advocate for their needs while working remotely in the Mountain Time Zone. Key responsibilities Oversee care management for a patient panel, utilizing nursing expertise and value-based care operations Build trusting relationships with patients and families to offer clinical and emotional support throughout their care journey Coordinate interdisciplinary care plans and monitor the effectiveness of care management strategies to improve patient outcomes Required qualifications Associate degree in nursing Current RN License required; Compact License preferred Experience in care management is required Certified Case Manager preferred Intermediate skills with MS Office Suite, including Outlook and Teams
