Base pay range
$37.00/hr - $40.00/hr

Contract : 9 Months (with potential for extension)
Pay : $37–$40/hour
Shift Start Times : Vary between 5:00 AM – 12:00 PM (Day Shifts Only but must be open to any start time)
Location : Remote (Must reside in a Compact State or have CA RN License)
Required: Compact license - BSN required
Preferred: Chronic disease management experience in outpatient settings (not required!)
Key Responsibilities
- Act as a care guide and advocate for patients navigating the healthcare system
- Collaborate in patient-centered care team meetings and quality improvement initiatives
- Deliver health and disease education and promote self-management practices
- Support care coordination with internal teams and external healthcare organizations
- Monitor post-hospitalization transitions (admissions, discharges, ER visits)
- Manage care for high-risk patients with chronic conditions and multiple co-morbidities
- Participate in preventive screenings and assist with day-to-day patient interactions
- Promote clear communication among care teams and providers
- Assist with medication management and refills under protocol guidance
- Support clinical audits, reporting, and evaluation of Virtual Care programs
- Collect and analyze biometric and health data for population health initiatives
- Provide clinical support and guidance to Medical Assistants and other care staff
Qualifications
- Required Licensure : Active RN license in a compact state or California (CA) license
- Compact RNs must reside in a compact state
- Willingness to obtain additional licensure as needed (facilitated by employer)
Education : Bachelor of Science in Nursing (BSN) required
Experience :
- 2–5 years in a clinical setting, ideally in outpatient care with chronic disease management
- Experience using Electronic Health Record (EHR) systems
- Proficient with Google Suite and other basic computer tools
