Remote Physician Collaborator for Senior Care

Humana's CenterWell Senior Primary Care seeks a Physician Collaborator for a remote role reporting to the Associate Director. You will work with APPs to provide wraparound services for patients who had fractures in the last six months when they can't see their own PCP.

Responsibilities include closing care gaps, performing chart reviews, coordinating monthly meetings, and serving as a resource for APPs to ensure high-quality patient care; travel to Humana offices may be required occasionally.
