ComplexCare Solutions (CCS) is seeking a Nurse Practitioner Per Diem to cover Hartford County, CT. The role involves in-home assessments with a focus on evaluating health status, gaps in care and risk factors to keep members in the home and out of the hospital.
Providers will spend up to an hour with members, documenting findings, coordinating with PCPs and health plans, and following CMS/HIPAA requirements. In-home visits and potential telehealth are available according to state needs.
