You will conduct phone visits to evaluate diagnoses, medications, and lab results, assess member acuity, and validate candidacy for our clinical program. In this contracted, remote role, you will identify and prescribe CGMs where appropriate, collaborate with RNs, RDs, and accountability coaches, and ensure compliance with Perry Health policies.
