What To Expect (Job Responsibilities)
- Build trusted relationships to empower patients in managing their health
- Engage with patients via calls, texts, and emails to facilitate care and address health challenges
- Collaborate with physicians, nurses, and care teams to enhance care coordination and outcomes
- Accurately document interactions in the electronic health record (EHR) for seamless communication
- Travel occasionally (10–15%) within Eastern and Central Pennsylvania for in-person engagements and retinal exams
What Is Required (Qualifications)
- Certified or Registered Medical Assistant or Certified EMT with 2+ years of experience
- Exceptional empathy, communication, and listening skills
- Ability to thrive in a remote work environment while collaborating with clinical teams
- Experience navigating EHR systems and Microsoft Office
- Strong attention to detail and ability to manage multiple priorities
How To Stand Out (Preferred Qualifications)
- Experience with specific EHR systems such as eClinicalWorks, Epic, Athena, or Cerner
