Sentara Health Plans is seeking a Manager, Integrated Care Management for Nursing Facility/ HCBS / LTSS Waiver. This remote role will lead a team of Care Managers supporting members in Nursing Facilities and Skilled Nursing Facilities, with 2 direct reports and oversight of 23 indirect team members.
The role demands a Bachelor’s degree, licensure (RN, LCSW, LCMHC, or LPC), and at least 3 years of case management experience with 1 year of leadership. LTSS/HCBS/Medicaid knowledge is preferred.
