Providing compassionate care remotely, the full-time Remote LCSW Mental Health Case Manager will conduct assessments, develop treatment plans, and coordinate care for patients and families facing behavioral health challenges. Key responsibilities Conduct behavioral health assessments and create individualized treatment plans Provide individual, family, and crisis intervention services while advocating for patient participation in treatment goals Coordinate care with interdisciplinary teams and maintain accurate clinical documentation in Epic Required qualifications Master's degree in Social Work, Counseling, or a related human services field Licensed Clinical Social Worker (LCSW) certification 2-3 years of behavioral health experience preferred Strong skills in assessment, crisis intervention, and care coordination Proficiency with Microsoft Office and electronic medical records
