Icon Health is seeking a board-certified physician to serve as collaborating physician for our Advanced Practice Provider team. The physician must live and practice in Kentucky and be available by phone or electronic means as APPs provide services.
The role supports licensing, credentialing, CAQH updates, payer enrollment, and clinical collaboration under the Practice Agreement. It is a remote, contract-based opportunity paid per APP, with a Monday–Friday schedule.
