Vaya Health is seeking an Innovations Care Manager to provide proactive intervention and coordination of care for eligible members across MH/SU/IDD/TBI and physical health domains. This mobile role requires residency in North Carolina and frequent travel to serve Cherokee County communities.
The Innovations CM conducts comprehensive assessments, supports interdisciplinary care teams, and ensures care planning meets NC DHHS requirements. Remote work is coupled with in-person contacts as needed.
