To support the management of heart failure patients, the full-time Remote Heart Failure RN will monitor vital signs, conduct virtual assessments, and coordinate care with the clinical team while providing patient education and support from a home-based environment. Key responsibilities Triage patients using established protocols to determine appropriate nursing measures and escalate care as needed Conduct virtual nursing assessments to evaluate patient symptoms, medication adherence, and disease progression Educate patients and caregivers on heart failure management, medications, and potential side effects Required qualifications Bachelor's Degree in Nursing Current, active, unrestricted RN license in a compact state Minimum of 3 years of experience in cardiology, heart failure management, or critical care nursing Strong background in heart failure patient management, including knowledge of Guideline-Directed Medical Therapy (GDMT) Advanced certification (CCRN, CHFN, RN-BC) is highly preferred
