Remote CCM Navigator: Expert Care Coordination

Unio Health Partners is seeking a CCM Navigator to organize, support, and implement our programs. You will perform non-face-to-face care coordination over the phone and communicate with physicians, staff, and patients to ensure effective, timely care.

Ideal candidates have a BA/BS (or BSN), clinical patient-care exposure, and 1+ year in a service role. Bilingual English/Spanish is preferred. This is a remote position available in eligible states.
