Working remotely on a full-time basis, the Remote Case Manager RN will manage comprehensive case management services for patients with chronic or catastrophic illnesses, ensuring high-quality, cost-effective healthcare while collaborating with healthcare teams and advocating for patient needs. Key responsibilities Conduct assessments to identify patients' medical and psychosocial needs and develop individualized care plans Facilitate communication among patients, families, and healthcare providers to ensure coordinated care and adherence to treatment plans Maintain accurate and timely documentation of case management activities and evaluate the effectiveness of care plans regularly Required qualifications Registered Nurse with a minimum of 5 years of clinical experience Active, unrestricted multi-state nursing license Preferred credentials such as CCM, CRRN, or OCN Recent clinical experience in a relevant healthcare setting Strong organizational and time management skills, with the ability to work independently
