Gentiva is seeking a Care Transition Coordinator to support transitions between care settings. This is a remote role with a Mon-Fri 4pm-12am schedule, focusing on referral coordination, chart reviews, and ensuring timely placement in the right care setting.
You will work closely with Care Transitions Nurses, educate patients and families on Home Health, Hospice, and Palliative Care, and participate in performance initiatives while maintaining HIPAA compliance.
