Experienced in navigating the VA system, the full-time Remote Care Navigator - VA Experience will assist Veterans with healthcare, benefits, and community resources while also enhancing care coordination workflows and supporting team training remotely. Key responsibilities Guide Veterans in understanding and accessing available healthcare and benefits resources Support complex Veteran cases by determining appropriate referrals and escalations Develop and improve Veteran-specific care coordination workflows and team training initiatives Required qualifications 5+ years of experience in care coordination, care navigation, or a related field with progressive program management experience Direct experience working with Veterans and/or within the VA system 1+ years of experience in a remote work environment Knowledge of VA healthcare, Veteran benefits, and behavioral health resources Experience documenting cases within a care management or electronic health record system
