Harbor is seeking a Care Coordinator to provide comprehensive, multi-system care coordination for youth with complex needs and their families. The role is full-time (40 hours per week) and remote in the community, with a preference for residents of Huron County.
The ideal candidate has 3+ years of experience in community-based services or an advanced degree, strong communication and computer skills, and the ability to work across family systems, case management, and child development.
