Collaborating closely with the RN Case Manager, the full-time Remote Care Coordinator will support members in closing care gaps and managing complex care needs while working remotely across the U.S.
Key responsibilities
- Reaches out to members to assist with referrals, authorizations, and appointment scheduling
- Creates cases and documentation in the Case Management module for hospital and SNF discharges
- Establishes relationships with members, acting as a patient advocate and managing alerts regarding changes in their condition
Required qualifications
- Minimum 1 year of experience in a healthcare setting, assisting members with authorizations and scheduling
- High School Diploma or GED; Bachelor's degree or four years of additional experience in lieu of education preferred
- Ability to communicate effectively and professionally with members and healthcare professionals
- Knowledge of Managed Care Plans and Medi-Cal
- Bilingual in English and Spanish is preferred
