Centene is seeking a Care Coordinator to support our Illinois Health Plan Medicaid YouthCare program members. This position is fully remote with occasional travel to local office for quarterly meetings; schedule is Monday–Friday, 8am–5pm CT.
You will engage members by phone or home visits, coordinate care plans with providers and partners, educate members about benefits, and document care needs in compliance with state regulations.
