The Coordinating Center is seeking a full-time Supports Planner to provide home-based case management for individuals with complex medical needs and/or disabilities in Maryland. You will visit client homes and community locations, coordinating services with clients, families, and providers.
You will develop person-centered plans, advocate for needs, and collaborate with nurses and social work consultants to resolve issues while maintaining a flexible schedule and high-quality care.
