Humana is seeking a Care Coach for a remote role in Georgia. You will evaluate member needs, guide families to appropriate resources, and develop personalized care plans to achieve optimal wellness. Training runs 4 weeks, then standard hours Monday–Friday with some travel for training or meetings.
The role requires an Associate degree with 2+ years in direct adult patient care, proficiency in EMR, and strong communication. A Bachelor's in Allied Health is preferred.
