Remote Cardiology Care Manager

CardioOne is seeking a Virtual Care Manager to join its Virtual Care Platform. This full-time role is remote and reports to the Manager of Virtual Care Services, supporting patients between office visits to keep them healthy and engaged in their health goals.

You will provide remote care management, including disease-specific care plans, medication reconciliation, education, and coordination with clinicians. You’ll work with LPN Care Coordinators and help onboard new team members.
