Remote Bilingual Patient Outreach Coordinator (Spanish)

Triplemoon is seeking a Patient Outreach Representative to manage scheduling for our CoCM program across a growing network of clinics. You will coordinate BHCM sessions and intake visits, helping families move from referral to enrollment while protecting a positive care experience.

You will work in a fully remote, part-time hourly role, partnering with the Behavioral Health and Clinic Success teams, learning billing codes as needed, and ensuring timely communication with families and staff.
