Providing virtual nursing services, the full-time Registered Nurse, Telehealth will conduct telehealth visits, perform clinical assessments, and offer member education for individuals with chronic cardiometabolic conditions in a remote capacity. Key responsibilities Conduct scheduled telehealth nursing visits and clinical assessments for members managing chronic conditions Provide nursing support and education through member messaging, addressing questions related to symptoms and care plans Document clinical encounters accurately in the electronic health record and communicate any clinical escalations to the appropriate providers Required qualifications Active, unrestricted Registered Nurse license in good standing, with a Nurse Licensure Compact (NLC) multistate license preferred Bachelor of Science in Nursing (BSN) is required A minimum of five years of clinical nursing experience, with at least one year in a telehealth or virtual care environment Experience supporting individuals with diabetes, obesity, hypertension, or dyslipidemia is strongly preferred Proficiency with electronic health records and technology supporting virtual member care
