The Winchester Post Acute unit provides care for patients transitioning from hospital to home. Services include post‑surgical recovery, chronic illness management, and rehabilitation, helping patients regain independence to return to the community. A multidisciplinary team of nurses, therapists, physicians, and support staff collaborates to deliver personalized care plans that support healing and emotional well‑being. The unit emphasizes patient education, family involvement, and continuity of care, making patients feel seen, heard, and empowered in their recovery.

How you'll contribute
- Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.
- Performs plan of care interventions, including medication administration, specimen collection, clinical treatments, and other medical care treatments.
- Documents patient care given.
- Monitors, records, and communicates patient condition as appropriate to the care team, physician, patient, and family.
- Assists patients with activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
- Collaborates across disciplines to coordinate patient care, including patient transfer, discharge, referral, and spiritual/psychosocial support needs.
- Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
- Works as an advocate for the physical and emotional well‑being of the patient.

What we're looking for
Applicants should have a current Tennessee or compact state RN license and possess an associate’s degree from an accredited nursing school.
- Certifications: AHA Basic Life Support (BLS) within 30 days of hire.
- Required Skills: Critical thinking, decisive judgment, ability to work with minimal supervision, and function effectively in a stressful environment.
- Experience: One year of previous nursing experience preferred.
- Licenses: Licensed RN in Tennessee or a compact state with proper Tennessee paperwork filed within 30 days of hire.
- Education: Associate's degree required; BSN preferred.
