Job Type and Compensation
Full-Time | 1.0 FTE or 80 hours per patient per month.
$5,000 Sign-on Bonus, subject to eligibility criteria.
Schedule
Monday-Friday, 8-hour day shifts.
About the Role
Clinic RN serving the community at UP Health System - Portage.
Responsibilities
- Perform plan of care interventions, including medication administration, specimen collection, clinical treatments, and other medical care.
- Document patient care provided.
- Monitor, record, and communicate patient condition to the care team, physician, nursing home, patient, and family.
- Collaborate across disciplines to coordinate patient care, including transfer, discharge, referral, and spiritual/psychosocial support needs.
- Evaluate learning needs of patient and/or family and provide education appropriate to age, culture, condition, and circumstances.
- Advocate for the physical and emotional well‑being of the patient.
- Delegate patient care activities to appropriately qualified staff.
- Perform prior authorizations on tests, procedures, medications, etc. required by the patient’s insurance.
- Perform other job duties as assigned.

Qualifications
- Current Michigan RN license (or attain within 90 days of hire).
- Associate degree from an accredited nursing school (bachelor’s degree preferred).
- Basic Life Support certification required (available upon hire).
- Annual influenza vaccination or declination required.
