- Manageable Caseload: 3–4 patient assignments, which may include admits/discharges, per 12-hour shift

What You’ll Do
- Deliveracute, hospital-level care in patients’ homes
- Perform skilled clinical tasks (IV starts, phlebotomy, medication administration)
- Monitor patient conditions andrecognize changes requiring escalation
- Collaborate with avirtual and in-person interdisciplinary team
- Use evidence-based practice toensure safe, patient-centered care
- Coordinate discharge planning and ongoing care needs
- Promote aculture of safety and high-quality outcomes
What We’re Looking For:
- ActiveRN license in the state of Illinois
- BLS certification (AHA)
- Valid driver’s license with a clean driving record
- 2+ years of recent acute inpatient care or ED experience preferred
- Strong IV, phlebotomy, and clinical assessment skills
- Ability to workindependently in home settings
- Excellent communication and critical thinking
- Comfortable withtechnology and remote monitoring tools
Work Environment
- Travel within designated service area
- Provide care inprivate home settings
- Physical activity required (lifting, bending, standing, etc.)
- Exposure to clinical and environmental variables typical in-home care
