The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care, developing and implementing patient care plans, and collaborating with families and healthcare professionals. They evaluate patient needs, initiate care strategies, and document progress, ensuring quality and holistic well-being across physical, psychosocial, emotional, and spiritual domains. The role involves conducting initial evaluations, reassessing patient conditions regularly, and guiding families in self-care techniques.
Key Responsibilities
- Perform initial nursing assessments within 48 hours of referral to determine hospice eligibility
- Identify and reassess patient and family needs at least every 15 days
- Develop and coordinate personalized care plans
- Document patient problems, goals, interventions, and responses
- Collaborate with patients, families, physicians, and interdisciplinary team members
- Educate and supervise families in self-care techniques
- Maintain accurate clinical documentation of patient conditions
Qualifications & Skills
- Current RN licensure in the practice state
- CPR certification or ability to obtain within 90 days
- Valid driver’s license, vehicle insurance, and access to reliable transportation
- Ability to work effectively in diverse home environments
- Preferred: 1+ years of clinical experience
Compensation & Benefits
Salary ranges from $71,640 to $107,460 annually, based on experience and location.
