Job Summary
The registered nurse plans, organizes, and directs hospice care with an emphasis on community health education and experience. As an RN Hospice, you will utilize your nursing skills to provide professional care and support to patients and their families within their homes and communities.
Duties & Responsibilities
Patient Care
- Complete comprehensive assessments of patients and families to determine hospice needs
- Develop and implement care plans based on nursing diagnosis and palliative nursing actions
- Administer medications and treatments as prescribed by physicians
- Assist patients with activities of daily living and promote self-sufficiency and comfort care
- Coordinate patient care as a Case Manager
Communication
- Maintain accurate clinical notes and records
- Communicate with physicians and community health-related persons to coordinate care
- Teach patients and families self-care techniques and provide instructions
- Provide a safe environment for patients
- Work with interdisciplinary group members to meet emotional needs of patients and families
Additional Duties
- Participate in on-call duties as required
- Ensure availability of necessary equipment and services
- Supervise ancillary personnel and delegate responsibilities when required
- Maintain and upgrade professional knowledge and skills through continuing education
- Actively participate in quality assessment performance improvement activities
Job Requirements
- Graduate of National League for Nursing accredited school of nursing
- Minimum of two years of nursing experience, preferably in public health, home care, or hospice
- Current licensure to practice professional nursing in the state
- Excellent observation, problem-solving, and communication skills
- Ability to organize and prioritize workload independently
- CPR certification
- Valid driver's license and insured automobile in good working order
- Self-directed and able to work with minimal supervision
