Overview

This role is for the nurse everyone relies on. Every team has one – the nurse who can walk into any situation, quickly assess, and bring calm and structure. As a Floating RN Case Manager, you step into different territories and help teams stay on track. You stabilize caseloads, support transitions, and ensure patients continue to receive consistent, high‑quality care.

What You’ll Do

• Step into territories needing support due to census changes or coverage gaps.
• Provide skilled nursing care, assessments, and care planning.
• Complete OASIS visits and coordinate interdisciplinary care.
• Help maintain continuity and quality across teams.
• Educate patients and families while building trust quickly.
• Travel throughout the metro with flexibility and independence.

What We’re Looking For

• Active Minnesota Registered Nurse (RN) license.
• Strong clinical judgment and ability to adapt quickly in new environments.
• Experience in home health preferred, but not required.
• Confident communicator and problem‑solver.
• Reliable transportation and willingness to travel.
