Role: RN-Case Manager providing in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses.
- Responsibilities: Conduct weekly in-home nursing visits, perform physical assessments, manage medications, ensure home safety, oversee caregivers, and support client health and well-being.
- Key Duties: Develop and evaluate care plans, collaborate with staff and regional leaders, participate in staffing and training, ensure documentation accuracy, and contribute to quality improvement initiatives.
- Qualifications: Valid RN license, BSN preferred, minimum two years nursing experience (including one in home care), CPR certification, and relevant supervisory or case management experience preferred.
- Skills: Strong clinical assessment, communication, collaboration, and organizational skills, with adherence to agency policies and scope of practice.
