Job Summary

Registered Nurse at Prevea plays a crucial role as an integral member of the clinic team, delivering patient‑centered care.

Responsibilities

• Manage patient/family health concerns through telephone triage and consultation.
• Actively seek information to understand patient circumstances, problems, expectations, and needs.
• Facilitate caller’s access to appropriate level of care by assessing needs and offering clinical options within scope of licensure.
• Refer to healthcare providers, services, and community resources as needed.
• Collaborate with other healthcare providers to develop and implement patient care plans to ensure best possible outcomes.
• Conduct telephone calls back to patients requiring follow‑up care or provide test results.
• Provide health counseling and education to patients and families about their healthcare and any follow‑up instructions.
• Provide direct patient care, including rooming of patients, obtaining medical information, performing comprehensive initial assessments, and maintaining patient charts/records.
• Assist physicians or advanced practice providers with procedures, document their instructions, and make necessary arrangements to ensure they are carried out.
• Administer medications and communicate information with patients, physicians, advanced practice providers, and staff to ensure quality and continuity of care.
